NURS-S 420 CARE COORDINATION IN TRANSITIONS OF CARE (3 CR.)
Students will synthesize knowledge and skills relevant to care coordination to ensure smooth care transition. Students will develop an understanding of the role of the RN as a member of a interprofessional team, as well as options for the most appropriate care setting for an individual patient.
